Single decision record
Decision memos are the internal record of human judgement.
They record what was reviewed, the outcome, supporting context and reviewer note. “Internal memo” is no longer a separate document type.
Alert versus decision memo
Alert: a system-generated signal that needs attention.
Decision memo: the reviewer’s recorded conclusion after checking that signal or transaction.
